Important Forms

New Clients

Interest Form - This interest form must be completed in order to find out some basic information about you and schedule a free "Meet & Greet" session.

Intake Form - Please fill out this form after you have had your free "Meet & Greet" session with D'Angelo or Nura.

Service Agreement Form - The Service Agreement outlines the expectations, policies, and responsibilities for both you and Fulcrum. It includes important information about services, fees, scheduling, confidentiality, and other guidelines to help ensure clarity and a smooth working relationship.

Release of Information Form - To protect your confidentiality, we must have your written permission before communicating with or sharing information with anyone outside of Fulcrum. If you would like your therapist to collaborate with doctors, school staff, or other providers, a completed Release of Information form is required.
