Our Intake Process
In order for Fulcrum to serve you best, this is our intake process:
1. Complete session interest form (found on the Interest Form page).
2. A staff member from Fulcrum All-Terrain Therapeutic Services, LLC will contact you to schedule and complete a FREE “Meet & Greet” session.
3. Once the “Meet & Greet” session has occurred, complete and submit the Intake Form (found on the Intake Form page).
4. After submitting intake packet, pay for first session by choosing the type and frequency of session package (found on the Payments page).
5. Once both the intake forms and payment have been received, the therapist will contact you directly to schedule first session.
